Request new DS-2019
Requesting a Form DS-2019 for a J-1 Scholar/Exchange Visitor to begin a new J-1 Program at the Miller School of Medicine:
  • Read the Exchange Visitor Program Policy and Procedure Manual for the Miller School of Medicine - click here
  • When identifying a candidate as a potential J-1 scholar, the following are the factors that should be considered: (1) qualifications, (2) eligibility for the position, (3) funding, (4) category, and (5) J-1 status versus H-1B status.  Departments should also consider any funded projects that the candidate would be assigned and whether there are any restrictions to dissemination of information or foreign national participation.  The Office of Research Administration will be able to assist with this inquiry and can be reached at (305) 284-3871.
  • In addition, if the foreign national will need to have access to export-controlled technology (such as high-energy-lasers, select agents or toxins, restricted radiation equipment, etc.), the Department considering the candidate should speak with the University of Miami’s Export Compliance Officer, Wendy Epley, who can be reached at (305) 243-9545.

    Also, all candidates must be screened through the University of Miami’s Restricted Party Screening (RPS) tool and cleared prior to any offer or invitation being extended.  Requests for screening must be submitted using Form EXPORT-F-006, which is available on the University of Miami’s Office of Export Control & Technology Management (ECTM) website: http://uresearch.miami.edu/regulatory-compliance-services/export-control/export-policies-forms The Department must complete the form electronically, submit it to ECTM, wait for ECTM  to return the form with the orange box on the bottom completed, and include it in the candidate’s packet that is submitted to ISSS for processing.

  • Once you have decided in which category you wish to invite the candidate to your department, complete the Form DS-2019 Request Form for the Miller School of Medicine - click here.
  • Also, issue an offer/invitation letter for prospective exchange visitors. For the template for the Miller School of Medicine Offer/Invitation letter - click here.
  • Submit the completed DS-2019 request form with all required signatures on page 4 and the offer/invitation letter with all required signatures to International Student and Scholar Services (ISSS), 5600 Merrick Drive, Bldg. 21-F, Coral Gables, FL 33124-5550 with the following attachments (all attachments must be written in English; if an attachment is not available in English, the attachment must be accompanied by a certified translation):
    • Prospective exchange visitor's curriculum vitae
    • Copy of approved position paperwork (only for prospective scholars with UM funding)
    • Proof of funding for all non-UM funding (proof of funding must be on letterhead and indicate the amount of funding the prospective exchange visitor will receive from the funding source during his/her time at UM; funding must be listed in U.S. currency).
    • Copy of the Cleared Screening Results Page: All prospective scholars must be screened through the University of Miami’s Restricted Party Screening (RPS) tool and cleared prior to any offer or intent letter being extended.  Requests for screening must be submitted to the University of Miami’s Office of Export Compliance through Form EXPORT-F-006, available on Export Compliance’s website: http://www.miami.edu/exportcompliance
  • Deadline for submission of all required documents listed above is no less than three months prior to the need for the exchange visitor's services.
  • Upon receipt of all required documents, ISSS will review the request for Form DS-2019. If the prospective exchange visitor is deemed eligible and qualified to participate in the Exchange Visitor Program, ISSS will issue Form DS-2019 for the prospective exchange visitor (and Forms DS-2019 for any accompanying dependent spouse and minor children) and send Form(s) DS-2019 to the prospective exchange visitor via FedEx, along with prearrival information, Form I-9 completion reminder (only for scholars with UM funding), Health Insurance Requirement Certification, and Form I-901.
  • The prospective exchange visitor is required to pay the Student and Exchange Visitor Information System (SEVIS) fee and visa fees prior to making arrangements for his/her visa appointment with a U.S. consular office abroad. Visa processing by a U.S. consular office can take anywhere from several days to several months, depending on the U.S. consular office and possible mandatory security clearances.
  • Provided the prospective exchange visitor obtains a J-1 visa from a U.S. consular office, he/she must enter the U.S. and attend exchange visitor orientation with ISSS within a certain period of time:
    • Up to 30 days before the expected program start date indicated initem #3 of Form DS-2019; and
    • No later then 30 days after the expected program start dateindicated in item #3 of Form DS-2019.
Form DS-2019 and the prospective exchange visitor's SEVIS record automatically deactivate if he/she does not enter the U.S. and attend exchange visitor orientation with ISSS within this time frame.

Extend current DS-2019
Requesting a Form DS-2019 for a J-1 Scholar/Exchange Visitor to extend a J-1 Program at the Miller School of Medicine:

  • Complete the Form DS-2019 Request Form for the Miller School of Medicine - click here.

If the prospective exchange visitor is deemed eligible for an extension of his/her J-1 program, ISSS will extend the J-1 program and issue Form DS-2019 for the exchange visitor (and Forms DS-2019 for any accompanying dependent spouse and minor children) and contact the exchange visitor via e-mail for an appointment with ISSS.

Requesting a Form DS-2019 for a J-1 Scholar/Exchange Visitor to transfer to UM's J-1 Program at the Miller School of Medicine from a different J-1 Program:

    • Copies of the exchange visitor's passport biographical data page, expiration date page, J-1 visa page, front and back of Form I-94, and Form(s) DS-2019 issued by the current and previous J-1 program sponsors.
  • Deadline for submission of all required documents listed above is no less than two months prior to the need for the exchange visitor's services.
  • Upon receipt of all required documents, ISSS will review the request for Form DS-2019. If the prospective exchange visitor is deemed eligible and qualified to participate in UM's Exchange Visitor Program, ISSS will contact the exchange visitor's current J-1 program sponsor and request that he/she be released to UM's J-1 program at the requested transfer release date. Upon notification by the current J-1 program sponsor that the transfer has been approved, ISSS will notify the exchange visitor via e-mail to visit ISSS on the requested transfer release date. Form DS-2019 for the exchange visitor (and Forms DS-2019 for any accompanying dependent spouse and minor children) will be issued during the appointment with the exchange visitor on the requested transfer release date.

Requesting an amended Form DS-2019 for a J-1 Scholar/Exchange Visitor at the Miller School of Medicine:

  • Complete the Form DS-2019 Request Form for the Miller School of Medicine - click here.
  • Submit the completed DS-2019 request form with all required signatures on page 4 to International Student and Scholar Services (ISSS), 5600 Merrick Drive, Bldg. 21-F, Coral Gables, FL 33124-5550 with the following attachments (all attachments must be written in English; if an attachment is not available in English, the attachment must be accompanied by a certified translation):
    • Copy of approved position paperwork (only for scholars whose current Form DS-2019 will be amended to add UM funding, and scholars whose current Form DS-2109 will be amended to reflect a change in UM position title and salary).
    • If you are requesting an amended Form DS-2019 to add non-UM funding: Proof of funding for all non-UM funding (proof of funding must be on letterhead and indicate the amount of funding the prospective exchange visitor will receive from the funding source during his/her time at UM; funding must be listed in U.S. currency)
    • DS-2019 Applicant Review Form (EXPORT-F-006), completed electronically by your department, submitted to the University of Miami’s Office of Export Control & Technology Management (ECTM), and returned to your department by ECTM with the orange box on the bottom completed.  All current scholars must be screened and cleared by ECTM prior to the extension of an on-going program.  Requests for screening must be submitted to ECTM through Form EXPORT-F-006, available on ECTM’s website: http://uresearch.miami.edu/regulatory-compliance-services/export-control/export-policies-forms
  • Deadline for submission of all required documents listed above is as soon as you become aware of the need for an amendment of the exchange visitor's current Form DS-2019.
  • Upon receipt of all required documents, ISSS will review the request for Form DS-2019. If the prospective exchange visitor is deemed eligible for an amendment of his/her J-1 program, ISSS will amend the J-1 program and issue Form DS-2019 for the exchange visitor (and Forms DS-2019 for any accompanying dependent spouse and minor children) and contact the exchange visitor via e-mail for an appointment with ISSS.
