Social Security Number (SSN) Verification Form for On-Campus Employment
Transfer Eligibility Form
7 Business Days:
Change of Educational Level
Change to F-1 or J-1 Student Status
Curricular Practical Training (CPT)
Employment Authorization Based on Severe Economic Hardship
Employment Authorization for J-2 Dependents
Employment Authorization (other than academic training) for J-1 Students
Internship with International Organization Employment
Optional Practical Training (OPT)
Optional Practical Training (OPT) STEM Extension
Reinstatement to F-1 or J-1 Student Status
- attend 2nd institution

- confirm valid F-1/J-1 international student status
