Transcripts cost $6.00 per copy mailed out and $7.00 per copy picked-up in person. Click here for PAYMENT INFO.

To be sure you provide all of the necessary info, please complete the Transcript Form along with an additional form for multiple requests. Please make sure to fill out the first form regardless of how many requests you might have. (Downloading the Transcript Form requires Adobe Acrobat Reader- a free download).

Simply complete the form, print, SIGN, and mail or fax it, with your payment, to:

We MUST have your SIGNATURE on all transcript requests; we cannot accept emailed requests.

 

Please note:  Once a transcript has been mailed, the recipient address will not be changed. If you would like a transcript sent to a different address, a new request must be submitted.

 

If you are not able to download our form, please be sure your written request includes the following information:

  • Number of copies ordered 
  • Student’s current name
  • Student’s name as it appeared on the student record if different from current name
  • Your C# (student ID number) and/or your social security number 
  • Date of birth 
  • Approximate dates of attendance 
  • Address where transcript is to be mailed 
  • Your address and phone number 
  • Any deadlines should be highlighted or underlined for emphasis 
  • Your signature 
  • Payment - check, money order or credit card (Visa and MasterCard only and include expiration date and billing zip code). No Cash.

 

Once we receive your request, it usually takes 3 to 5 business days to process. Please be sure to account for mailing times both for us to receive your request and for the post office to deliver.
