The Office of the Registrar maintains student academic records and can assist you with updates to your biographical/demographic records. This includes changes to your name, social security status, gender, marital status and citizenship.
Change of Name
Students who need to have their official name changed must fill out the Bio/Demo Change form and bring proof along with a photo ID (Passport or Driver’s License) to the Office of the Registrar.
Documents verifying a name change must be original, legal documents or certified copies.
Acceptable documents are:
Marriage license
Divorce document
Naturalization papers
Passport
Court Order
*Birth Certificate
*(Birth certificates are acceptable only for correcting spelling errors.)
To change a married name back to a maiden name, legal documentation other than a birth certificate must be provided.
International Student Name Changes
In addition to providing the above documentation, International students must also first obtain a signature on the Bio/Demo Change form from their International Student and Scholar Services (ISSS) advisor. ISSS is located in Building 21, Room F, 3rd floor. Once the signature is obtained, the form may then be submitted with the legal documentation to the Office of the Registrar.
Change of Social Security Number
Students who need to update or correct their social security number must provide documentation to the Office of the Registrar. This can be done by completing the Bio/Demo Change form.
International Students
International students who are NOT able to obtain a social security number
Change of Gender Status
If there is an error on your record or you would like to make an update to the gender recorded for you in the student information system please request this change in writing and provide appropriate documentation to the Office of the Registrar. Students who wish to change their gender status must complete the Bio/Demo Change form.
Acceptable documents include:
*The Office of the Registrar follows the individual state policies listed on the National Center for Transgender Equality website. If the state issuing the presented driver’s license is not listed on this site, your request may require further review.
Change of Citizenship
Students who need to update or correct their citizenship must request this change in writing and provide documentation to the Office of the Registrar. This can be done by completing the Bio/Demo Change form.
Documents verifying citizenship must be original, legal documents or certified copies.

Please note, changes in citizenship other than U.S., may require an additional process through the International Student Scholar Services (ISSS) office.
Change of Marital Status

Documents verifying change in marital status must be original, legal documents or certified copies.
