FMLA Leave of Absence
Eligible faculty and staff can request a leave of absence for medical and personal reasons specified in the University’s Family and Medical Leave policy. An employee requesting FMLA leave is required to complete and submit this request form through the University’s Workday system, or directly to the designated leave coordinator, if the employee is unable to submit the request through Workday. Employees should consult the University’s Family and Medical Leave policy for information on the timeframe for submitting such requests and other information concerning an employee’s FMLA leave. Should an employee have any additional questions regarding the policy, please consult your assigned HR Partner.
