Probationary Period Appraisal Request Form

Probationary Period Appraisals for New Hires, Transfers, Reclassifications (promotion, lateral or downward) are not required but highly recommended. Probationary Periods are defined here. To request a probationary period appraisal for a NON-FACULTY/NON-STUDENT/REGULAR employee of the Coral Gables or Marine campus, please complete the form sections below. You will be notified via email when the appraisal is available for you to complete.

Identifies the 9 character Employee ID. Example: C12345678

* Title of Employee to be Evaluated (You must type in a response)

* Job Code of Employee to be Evaluated (You must select one)

Indicates the new job code of the employee to be evaluated.

	H (Non-Exempt Hourly)
	I (Non-Exempt Hourly)
	J (Non-Exempt Hourly)
	K (Non-Exempt Hourly)
	R (Exempt Research)

* Department Number of Employee to be Evaluated (You must type in a response)

Identifies the 7 digit dept/sub-dept number of the employee's new assignment. Example: 44005-00

* Department Name of Employee to be Evaluated (You must type in a response)

* E-Mail Address of Employee to be Evaluated (You must type in a response)

* Effective Date of Hire/Transfer/Reclassification (You must type in a response)

This is the first day of the new hire/transfer/reclassification

* Name of Supervisor (You must type in a response)

This field identifies the name of the supervisor of the employee to be evaluated. This is the name that should appear as the supervisor to the employee in DHRS.

* Name of HR Liaison (You must type in a response)

* C# of HR Liaison (You must type in a response)

Identifies the 9 character HR Liaison ID. Example: C12345678
