Certain changes to a document require re-approval and others don’t, as defined by Human Resources when the forms were designed. If you have additional fields that you think should require re-approval if changes are made, please suggest them and we will consider adding them to the list. In addition, it is important to consider that if a person edits and saves a document, it will stay with the current approver, unless the change made requires re-approval. If the document is withdrawn and re-submitted, it will always go through the entire approval process again.

After you receive an approval from your recruiter, complete the top part of the Background/Scholastic Verification Form and email the form to your new hire. The new hire must complete the form and fax it to HR at 305-284-2854. HR will arrange for drug testing kit delivery to the new hire’s location.

The department needs to send the Background/Scholastic Verification Form to Human Resources. The new hire needs to report to HR within 48 hours of the job offer for the drug test. After the drug test is completed, the new hire working at the Gables Campus needs to report to Faculty Affairs to complete additional paperwork. RSMAS Campus Employees ONLY: need to report to the Dean’s office at the Rosenstiel Campus.

Yes, a new Background/Scholastic Verification Form is required when a position is filled by either a new or existing employee in order to appropriately update the employee's file.

Yes, a layoff employee from any job class can be hired as an Administrative/Professional TT or CW working layoff. The required employment forms must be completed by the employee if they are missing in the employee's file. The hiring department must submit a Background/Scholastic Verification Form along with the job description.

All employees rehired after 30 days of separation must be treated as new hires and complete new W-4 and Direct Deposit forms.

If rehired within 30 days, then the tax information is unchanged, thus the employee does not have to complete a new W-4 Form. Apart from this exception, all rehires have to be treated essentially as new hires and complete new W4 and Direct Deposit forms.
