There are three ways to receive the credit:
1. From January 9 - 30, 2012, visit
www.blueprintforwellness.com to schedule an appointment for an on campus screening in February. Click "I'm in a workplace program," enter the Registration Key (the key is
UM), and enter your Aetna ID without spaces including the "W" (your ID is located on the top left of your Medical ID Card).

* If you participated in the event in 2011, you must enter your user name and password. If you do not remember your user name/password, please click Forgot Username or Password, enter your Registration Key (the key is UM), Unique ID (Aetna ID, including the W, without spaces) and Date of Birth.
2. By January 30, 2012, visit
www.blueprintforwellness.com and indicate you are requesting an appointment at your local Quest Diagnostics Patient Center (Attend a Quest Patient Service Center at your convenience). We recommend allowing 24 hours between the date you make the appointment and the actual appointment. This allows the Quest systems to be updated. Please print your confirmation and take it with you to the lab. You must complete your testing by March 31, 2012.
3. Before March 31, 2012, report recent blood work (from August 2011 through March 2012) results at
www.aetna.com.
Note: If you log onto www.aetna.com, your blood results may already be in the system. You MUST click through the entire "Walk Me Through" portion of your Personal Health Record, as outlined below, to receive the $100 credit for the incentive, even if your blood results are already loaded.
To participate:
- Visit
www.aetna.com.
- Log in, or click “Register,” to create a username and password.
- Click "Health Records” and then select “Personal Health Record.”
- Click and answer questions for all the pages in the “Walk me Through” site:
• Personal Information
• Emergency Contact
• Insurance
• Health Team
• Immunizations
• Family History
• Medications
• Allergies
• Conditions & Symptoms
• Tests & Procedures
• Hospital Visits
- When you reach the “Tests and Procedures,” screen, click the drop down box to see all of your current health records.
• If you are missing recent (August 2011 through March 2012) lab results for: Total Cholesterol, HDL Cholesterol, LDL Cholesterol, Triglycerides and Blood Glucose (please look for Finger Stick Value), click “Add New Test.”
• Add the appropriate tests and results. All results must be numeric. Save your data.
- Finish clicking through the “Walk Me Through” page.
- Once you reach the “Congratulations, you have completed your Personal Health Record,” screen you are done.
Note: You MUST click through the entire "Walk Me Through" portion of your Personal Health Record to receive credit for the incentive, even if your blood results are already in the system.
