All students must bring a Certificate of Defense Approval form to the defense for signing by each committee member. This form is your proof that you successfully defended your thesis or dissertation. Please download, fill out the form, and bring the completed form to the defense for your committee members to sign after your successful defense. The completed form must be submitted to the Graduate School with the two other final forms required to complete your ETD submission after you have uploaded a final PDF to the ETD database. Only original signatures on the form will be accepted by the Graduate School.

Some committees allow students to bring the Signature page from the thesis or dissertation for signing after a successful defense. If your committee allows this, use the Signature page template in the front matter template to format this page. It is recommended that students e-mail the page to the dissertation editor before the page is signed to ensure that the Signature page is formatted correctly. Committee members sometimes refuse to sign the Signature page if the title or other information on it is inaccurate. If you are asked to revise the page you will have to re-obtain signatures from your committee members at another time.
