Social Security Number Application


  • Read the handout on Applying for A Social Security Numberclick here.
  • Complete Form SS-5, Application for Social Security number, available at the Social Security Administration office or online at www.ssa.gov
  • In addition, F-1 students must also provide the Social Security Administration office with the following documents:
    • Two forms of identification, including your passport and an additional form of identification.
    • Forms I-20 and I-94.
    • Proof of employment authorization:

    For On-campus Employment:

    • Offer of on-campus employment from on-campus employer (see sample letter in handout).
    • Letter from ISSS Advisor granting on-campus employment.

    For Curricular Practical Training (CPT):

    • Form I-20 with CPT approval on page 3.

    For Optional Practical Training (OPT):

    • Form I-20 with OPT approval on page 3 and Employment Authorization Document (EAD), issued by U.S. Citizenship and Immigration Services.

    For Employment Authorization based on Severe Economic Hardship:

    • Form I-20 with employment authorization approval on page 3 and Employment Authorization Document (EAD), issued by U.S. Citizenship and Immigration Services.
  • J-1 students must also present the following documents to the Social Security Administration office:
    • Two forms of identification, including your passport and an additional form of identification.
    • Forms DS-2019 and I-94.
    • Proof of employment authorization.
