• Contact your International Student and Scholar Services (ISSS) advisor to discuss your options for replacing a lost Form I-94.

• If it is determined that it is better for you to apply for a replacement Form I-94 than to obtain a new Form I-94 upon your next entry to the U.S., then do the following:

• Complete Form I-102 Application for Replacement/Initial Nonimmigrant Arrival-Departure Document - click here.
• Purchase a money order in the amount of $320.00, made payable to the Department of Homeland Security.  Money orders can be purchased at U.S. Post Offices, Publix supermarkets and banks.

• Submit the completed Form I-102, money order in the amount of $320.00, and any supporting documentation as discussed with your advisor to ISSS. Your advisor will review and submit your application to U.S. Citizenship and Immigration Services (CIS).

• Please allow three business days for processing of your application for replacement of your Form I-94. CIS takes at least three to four months to adjudicate this type of application.

• ISSS will notify you once the receipt notice for your application for replacement of Form I-94 has arrived so that you can collect the receipt notice. The receipt notice includes your application receipt number on the upper left side of the page. To check the status of your application, please go to www.uscis.gov and click on Case Status Online on the left side of the screen. Click on Finding the Status of Your Case and type in your application receipt number to find out the status of your application.

• ISSS will subsequently notify you once CIS has sent a replacement Form I-94 so that you can collect the document from ISSS.
